Erectile dysfunction (ED) or impotence is a health, sexual problem that affects men of all ages. Men affected by ED are unable to develop or maintain an erection sufficient for intercourse despite being sexually stimulated. Most cases of ED are vascular in origin.

It often happens that the problem of ED is combined with a psychogenic component. Namely, it is culturally established that the sexual act of maintaining an erection is an act of masculinity, and if that part fails, it negatively affects the psychological aspect of the man. He begins to create a bad image of himself and his quality of life deteriorates significantly. It should be emphasized that sexual arousal is a complex mechanism that includes the brain, feelings, hormones, blood system, muscle and nervous system. An erection is the hardening of the male genital organ due to sexual stimulation. The process consists of several stages. The arteries of the penis relax and expand, resulting in a greater flow of blood. As they expand and harden, they put pressure on the veins that drain the blood, so that more and more blood is introduced into the penis, and less and less comes out, thus the penis is getting firmer, and the erection is getting stronger. The quality of an erection is reflected in the ability to achieve an erection, its strength and its maintenance, so that both partners are satisfied during sexual intercourse.

An erection is a physical manifestation of sexual desire that eventually leads to ejaculation, which represents the climax of the sexual act when the seminal fluid is ejected. Erections can exist without libido, such as nocturnal erections or morning erections.

Erection begins with fantasies or stimulation of some senses (touch, smell, hearing). Through nerve pathways, these impulses are transmitted via the spinal cord to the penis, where the penis is filled with blood through the corpora cavernosa. While on the other hand, with the help of special mediators, the valves for blood return to the body are closed so that the penis is full of blood, which ensures its firmness during intercourse.

The problem of erectile dysfunction is most often a consequence of problems:

In practice, it has so far been shown that the problem, apart from the elderly, can also affect the middle-aged population, where the first symptoms appear around the age of 35. ED can lead to a decrease in the quality of life, the breakup of a partner relationship, a decrease in self-confidence, anxiety, depression.

The current way of treating erectile dysfunction includes pharmacological stimulation (PDE5 - phosphodiesterase type 5 inhibitors), the use of penile prostheses, vacuum pumps for the penis.

In order for a person to be considered to have a problem with ED, it must have been present continuously for three or more months.

Causes and Therapy

The condition of ED can be caused by a physical or psychological cause. One of the main causes of ED is restricted blood flow to the penis - the amount of blood in the corpora cavernosa is not sufficient to cause an erection. This type of ED is called vascular erectile dysfunction. Men with ED are often treated with oral medications. However, they are not the only way to treat erectile dysfunction. Now, one of the reliable ways of treatment is extracorporeal shock wave therapy (ESWT).

Shockwave therapy

Since 2010, shockwave therapy has been used to treat vascular erectile dysfunction.
